Aim: To describe a treatment approach that allows a Class I face to be obtained without regard for patient cooperation. This approach uses the Herbst appliance and allows a more predictable treatment outcome in the majority of patients. Methods: Herbst appliance use in orthodontic treatment, including cases of open bite, brachycephaly, and congenitally missing second premolars, is discussed. The case records of patients receiving Herbst therapy are reviewed. In addition, the protocol of Herbst therapy in 1 orthodontic office is detailed. Results: It appears the Herbst appliance can create an orthopedic effect in some individuals and induce skeletal changes greater than those attributable to growth. Over an extended period of time, the final pattern emulates nonorthodontic normal subjects. Conclusion: The Herbst appliance permits control of all orthopedic factors involved in the effort to create a Class I face, without dependence on patient cooperation. World J Orthod 2000;1:32–44.
