Aim: To illustrate the results achieved in a read and butter patient treated with the extraction of four first premolars and followed over more than a 20-year period. This patient was treated according to the classic Tweed nonexpansion concepts. The Tweed goals and precepts were adhered to, as were the mechanics. Method: The pretreatment records, posttreatment records, and 21 years postretention records of a male with Class I bimaxillary protrusion and moderate crowding are presented. Conclusion: This patient illustrates the facial and dental esthetics, occlusion, periodontal health, and stability that can be obtained with a proper diagnosis and the precise achievement of evidence-based treatment objectives.
