Aim: Many individuals with spacing in the maxillary anterior region, due to median diastema, congenitally missing laterals, or pathological migration, desire esthetic improvement of these anomalies. Methods: The orthodontic treatment and results of typical cases are reviewed, with discussion of the benefits and problems associated with treatment alternatives. Conclusion: The appropriate therapy varies widely, from orthodontic treatment alone to advanced orthodontic-periodontic-prosthodontic measures. The aim for the orthodontist in this interdisciplinary treatment is to move the teeth into positions allowing the best possible esthetic and functional results.
