This patient presented with pain in the right maxilla. The maxillary right first and second molars were nonvital and sensitive to percussion. The right infraorbital nerve was sensitive to palpation. The periapical radiograph (Figs 1a and 1b) reveals periapical radiolucencies bordered by a slight lamina dura. The frontal tomogram of the right maxillary sinus (Figs 2a and 2b) demonstrates considerable swelling of the mucosal membrane adjoining the periapical infection of the right second molar.
