Regulation of BCG-Polysaccharide Nucleic Acid and Dexamethasone on Th1/Th2 Cytokines from Peripheral Blood Mononuclear Cells in Oral Lichen Planus

Objective: To investigate the regulation of bacillus Calmette Guerin-polysaccharide nucleic acid (BCG-PSN) and dexamethasone on Th1/Th2 cytokines from peripheral blood mononuclear cells (PBMC) in oral lichen planus (OLP). Methods: PBMC were isolated from OLP patients and healthy controls by density gradient sedimentation. PBMC taken from patients with OLP were stimulated with phytohemagglutinin (PHA), PHA and BCG-PSN, PHA and dexamethasone, respectively for 72 h. The concentrations of interferon-gamma (IFN-gamma) and interleukin-4 (IL-4) in culture supernatants were determined by ELISA. The mRNA levels for IFN-gamma and IL-4 were evaluated in culture cells using semiquantitative reverse transcription-polymerase chain reaction (RT-PCR). Results: Compared with healthy controls, the levels of IFN-gamma in OLP patients were significantly lower (P < 0.05). The levels of IL-4 were higher, but not statistically significant (P > 0.05). The ratios of IFN-gamma to IL-4 were lower (P < 0.05). BCG-PSN increased significantly the level of IFN-gamma, but had no effect on the production of IL-4. In contrast, dexamethasone significantly inhibited the levels of IFN-gamma and IL-4. IFN-gamma production was inhibited significantly more than IL-4. Conclusion: Th2 immune response is predominant in OLP. BCG-PSN induces a switch from Th2 to Th1 cytokine. Dexamethasone is a broad range immunosuppressant inhibiting Th1/Th2 cytokines. Both drugs have a markedly different effect on cytokine production.
