Stereologic Analysis in Oral Precancerous Lesions and Squamous Cell Carcinoma

Objective: To analyze the spinous cells of the oral epithelium by stereologic method, and to determine the malignant process in the quantitative change of the nucleus. Methods: Forty-eight specimens were taken from the oral mucosa. These were comprised of normal mucosa, oral submucous fibrosis, leukoplakia, epithelial dysplasia, and squamous cell carcinoma. The measurements were made by an interactive image analysis system (IBAS-II, OPTON). The stereologic parameters were calculated. Results: Nuclear volume density and surface density increased steadily from normal mucosa, through lesions, to carcinoma (P < .01); cytoplasmic volume density, cell mean volume, and mean diameter decreased progressively (P < .01 or P < .05). Four parameters showed significant differences when the group of carcinoma was compared with the other four groups (P < .01 or P < .05). Conclusion: Stereologic parameters could objectively reveal the biologic and morphologic features during malignant development, explain the malignant process from the quantitative change of nuclei from normal mucosa, through lesions, to carcinoma, and distinguish carcinoma from other lesions.
