The sagittal ramus osteotomy of the mandible is the most frequently performed orthognathic surgical procedure. Stabilization of the osteotomy may be accomplisheed by rigid internal or wire fixation. This article illustrates lingual nerve injury following a bilateral sagittal ramus osteotomy of the mandible and fixation with a superior border wire. The etiology and surgical management of this injury are described.
