The accuracy of maxillary repositioning with Le Fort I osteotomy depends on the ability to duplicate the movements predicted by the surgical cephalometric prediction tracing and definitive model surgery. We have developed a simple technique which uses a custom maxillary acrylic template that is indexed to the occlusal surfaces of the maxillary dentition, allowing the accurate transfer of reference lines and the level and angulation of the maxillary wall osteotomy from mounted dental casts to the patient.
