A rigid adjustable pin (RAP) system is described which the author has utilized for four years for maxillary stabilization in superior, inferior, and anterior repositioning of the maxilla after osteotomy. Data from 31 surgical cases with a minimum follow-up of 16 months are analyzed for long-term stability and complications. Recommendations are made for the utilization of this system. When placed properly, the RAP system provided extremely stable and rigid fixation for the maxilla during the early postsurgical healing phase. Long-term stability of the maxillary osteotomies was found to be excellent.
