Screw fixaton following sagittal ostetomy of the mandible has become increasingly popular. This paper represents the authors' experience with and initial 60 cases of transoral placement of self-threading screw to stabilize the proximal and distal segments ( of the mandibular ramus) following sagittal osteotomy. The most critical steps in this technique appear to be positioning the proximal segmant of the mandible to maintain the proper relatioship to the glenoid fossa, and preserving mandibular ramus position.
