Various treatment options are available to manage excessive gingival display during smiling, ie, a gummy smile. This case report demonstrates the successful management of excessive gingival display via the combination of gingivoplasty and lip repositioning surgery. The lip repositioning technique is described in detail. The treatment resulted in reduced gingival display and an esthetically pleasing smile. Lip repositioning surgery is a promising technique to improve esthetics in the maxillary anterior sextant. (Am J Esthet Dent 2012;2:162–169.)
