Internal bleaching is a common treatment for discolored teeth, especially those discolored due to dental trauma. Although research shows that in-office internal bleaching with highly concentrated hydrogen peroxide can cause external root resorption in traumatized teeth, many clinicians still use this technique. This review and case report evaluates the use of internal bleaching in young patients with teeth discolored by trauma. The indications and techniques for internal and external bleaching of nonvital teeth are discussed. Further, the treatment of a young patient with an incisor darkened by trauma and endodontic treatment is described. After the treated tooth demonstrated resistance to external bleaching, a new approach was initiated using 35% hydrogen peroxide applied both internally and externally over three application periods of 10 minutes each.
